Samples for analysis and formulae for approval sent to the departmental laboratory are to be accompanied by form Y 15, Request to Laboratory for analysis.

COMPLIANCE
1. (1) Form Y 15, Request to Laboratory for analysis, is to be completed in quintuplicate (4 whites and 1 yellow) for distribution;
a) in the case of samples for analysis
(i) 3 white, the yellow and the sample(s) to
Revenue Canada, Customs and Excise,
Laboratory and Scientific Services Division,
79 Bentley Avenue,
Ottawa, Ontario.
K2E 6T7
(ii) 1 white copy for retention by the shipper.
b) In the case of formulae for approval
(i) 3 white and the yellow to
Department of National Revenue,
Customs and Excise,
Laboratory and Scientific Services Division,
Ottawa, Ontario.
K1A 0L5
(ii) 1 white copy for retention by the shipper.
(2) Y 15’s should be secured in an envelope without folding and forwarded by mail or, in the case of samples for analysis, they may be attached to the exterior of the package containing the sample(s).
2. Form Y 15 will be replaced in the near future by Form Y 15a, to be used for excise submissions to the departmental laboratory; however, distribution of the Y 15a will continue as prescribe in section 1.
3. From Y 15 or Y 15a may be obtained from Regional Excise Offices.
Regional Directors are requested to ensure that a copy of this Circular is forwarded by registered mail to all excise duty licensees concerned who are located in their regions.
