T3 slip
The T3 slip, Form T3, Statement of Trust Income Allocations and Designations, is used to identify beneficiaries, and to report amounts such as income and credits that the trust designates to them.
The T3 slip has three individual slips printed on each page or sheet. These are intended to be used for laser or ink jet printers, for typing, or to be filled out by hand.
